Distinguished Achievement Awards
The Distinguished Achievement Award honors an individual who has established a meritorious career record in a specific field of expertise in any area of design or technology in the performing arts or entertainment industry. One Distinguished Achievement Award per Commission per year is allowed.
How to Nominate
Before you begin to complete the online form below you will need the following information:
- Information about the person you are nominating, name, address, e-mail, and phone number.
- You must have available for upload the following documents regarding your nominee; Letter of Nomination, Bio, Resume/CV, letter/memo of support from the appropriate Commissioner, and two Supporting Documents.
- Nominator name, address, e-mail, and phone number. Nominator must be a member of USITT and enter a membership number.
- Do not click on the submit button until all information is completed and all documents are uploaded.
Deadline to nominate: July 15, 2011.
