Print application and submit by mail or fax.
Faxed applications only accepted with credit card payments or purchase orders. Check payments must be mailed.

Forms of Payment

USITT accepts Mastercard, Visa, Discover, and American Express. Purchase orders are accepted from academic or government institutions only; copy of purchase order must accompany membership form. Payment by check should be made payable to USITT in US funds and drawn on a US bank.
