Golden Pen Award Nomination Form
This page is the way USITT members nominate a book for the Golden Pen Award.
It is necessary to supply information about yourself, information about the book
being nominated, and a statement describing the book and why it
should receive the Golden Pen Award. Required fields are marked with a *.
Please remember to push the submit button at the bottom of the page
when finished. The information on this form will be e-mailed to
the nomination committee and an acknowledgement should be received
within a day or two.
If you have any questions about this form, or about the Golden Pen Award, please contact Lance Brockman, chair of the selection committee. Nominations close on
August 15
each year for presentation at the subsequent conference.
